What is included today
This directory scans the complete CMS NPPES November 2, 2026 monthly file, without API result caps. It includes active individual NPI records with qualifying physician specialty codes and a primary practice address in the 50 states, DC or Puerto Rico. Records are deduplicated by NPI. Secondary practice locations are not included. Some records appear in related directories.
What is not verified
An active NPI record does not establish an active medical license, current practice, board certification, availability, insurance participation or specific services. Most listings still contain registry information only. A small, clearly identified first batch adds summaries from official practice websites, documented organization roles and selected publications. Profiles also show available 2024 Medicare service activity from a complete CMS file scan matched by NPI. Each enriched item shows its source and date; source review does not establish current privileges or licensure.
